Application Form

You must be nominated to apply for a Senior Recognition Award. After being nominated, you will receive an email from srawards@iu.edu congratulating you on your nomination and encouraging you to apply by completing the form below.

Collect all of your application materials before you begin.

Please note that you will not be able to stop and restart the application.

Essay questions are available on the individual award pages: Wells, Stahr, Goodbody, Bulter, Mueller, Hill and Student Employee Award.

All students will be informed of their nomination and asked to complete the online application available Tuesday, October 15.

Applications are due Monday, December 2.
